58 yrs old patient, attended ED / ER with sepsis and feeling unwell and treated for sepsis.
He right leg cellulitis and was treated with IV antibiotics, initial CRP was 300, WCC 22. He did not respond well to antibiotics and became unwell with respiratory distress and acute pulmonary oedema. He was eventually intubated and ventilated 3 days later requiring inotropic support.
His only medical problem was type 2 Diabetes
His blood culture grew group B Streptococcus and therefore antibiotics changes to Benzyl Penicillin
His bloods tests have gradually improved with WCC down to 11, CRP 20, Hb 80 gm, eGFR >90
His echocardiogram showed severe MR, large vegetation on mitral valve leaflet, moderate aortic stenosis and possible severe TR.
