Hello everyone, i promised that i will be doing something for #air-clinic finally its here as promised.
Am not a medical student or a practictioner but i have a special zeal for medicine so i love to talk medicine read medicine but i dont practise it.
When i joined steemit i met who happens to be a medical doctor, dorctor george as we joyfully call him, this gave me hope that atleast i now have a medical doctor as my friend we can talk about medicine all the time, he is a busy man now and i dont think there will ever be a time for us to talk about medicine but i know am gonna meet him one day, since we are in the same geographical location it shouldn't be so hard to meet him afterall, maybe if there is some occassion, lets say #steemit-nigeria convention.
Well thats all about my infatuation with medicine, lets go to the real topic, i will be shedding more light on sublingual and buccal method of drug administration.
what is buccal drug administration
Buccal can can be defined as the area pertaining to the sides of the mouth or to the mouth: oral.
Relating to the cheek, density directed toward the cheek, also:
#buccal-cavity: refers to the oral cavity bounded by the lips, cheek and gums
Also Vestibule of mouth
Buccal administration involves placing a drug between your gums and cheek, where it also dissolves and is absorbed into your blood.
Also we have
- Pre-buccal
- Post-buccal
what is sublingual drug administration
#sublingual refers to substance situated or applied under the tongue
wikibook
Sublingual (abbreviated SL) from the latin word "under the tongue," refers to the pharmacological route of administration by which substance diffuse into the blood through tissues under the tongue.
wikipedia
Some medicines can be given sublingual, which means "under the tongue". The reason for this method of administration is that some medications can enter the bloodstream better or faster this way.
For example, B12 is a vitamin that for decades was given as an injection because when someone has Pernicious Anemia they cannot absorb it from the intestines due to a lack of a chemical called intrinsic factor that "grabs" it and pulls it into the bloodstream. A few years ago it was noted that if given sublingual, it would be adequately absorbed, eliminating the need for an injection.
Another example is when a patient in Hospice care is near death, unable to swallow but having pain, the pain medication can be given as a liquid under the tongue, where it is absorbed quickly and gives rapid pain relief.
A pill designed to be given orally will rarely be able to be administered sublingually. A pharmacist can tell you if a certain medication has a coating and absorption profile that will allow this form of administration.
An example of a pain medication intended to be taken orally that can be taken sublingually, if you don't mind a slight bitter taste, is hydrocodone. Approximately 1/3 of an oral dose has the same effects when taken sublingually. This method of sublingual dosing has been tried when a patient's Dr. does not prescribe enough of the medication in quantity to be taken orally to subdue the patient's severe pain.

Both buccal and sublingual is categorizes under mouth administration of drugs.
Many drugs are designed for sublingual administration, including #steroids, #cardiovascular-drugs, #barbiturates, #benzodiazepines, #opioid-analgesics, #enzymes and increasingly #vitamines-minerals.

Your doctor may prescribe sublingual or buccal drugs under any of the following circumstances:
A. The drug needs to get into your system quickly
B. You have trouble swallowing medication
C. The medication doesn’t absorb very well in the stomach
D. The effects of the drug would be decreased by digestion
E. The cheek and area under the tongue have many capillaries, or tiny blood vessels. There, drugs can be absorbed directly into the bloodstream without going through your digestive system.

ADVANTAGES OF BUC & SL
Sublingual or buccal forms of drugs have their advantages.
Because the medication absorbs quickly, these types of administration can be important during emergencies when you need the drug to work right away, such as during a heart attack.
Further, these drugs do not go through the digestive system, so they aren’t metabolized through your liver. This means you may be able to take a lower dose and still get the same results.
Another advantage is that you don’t have to swallow the drug. Drugs that are absorbed under the tongue or between the cheek and gum can be easier to take for people who have problems swallowing pills.
In this route of administration the liver is bypassed , and bioavailability is higher. This type of administration has rapid absorption.
DISADVANTAGES OF BUC & SL
On the other hand, sublingual and buccal drugs also have some disadvantages.
- Eating, drinking, or smoking, can affect how the drug is absorbed and how well it works.
- Also, these forms don’t work for drugs that need to be processed slowly by your system, such as extended-release formulations.
- Any open sores in your mouth can also become irritated by the medication.
- Holding the dose in the mouth is an inconvenience . If any of the drug is swallowed it will then be subject to the fpe
