DESCRIPTION : at about 12:01 at PPU TS6 top elevation platform the structural team both Sub-contractor Absalaj and OCFZE Structural were engaged in installation of structural beam and tacking of handrails at height of about 12meters when a grating material weighing about 20 to 25kg loosed it bearing were it was placed.and Absalaj team were standing on the loosed gratings to work,
(height from the location to were the above mentioned name was standing was about 2meters) the grating fell on his back and he sustain a cut. and he wasn't able to move .....
Note from the little investigation Carried out we discovered that most of the gratings were placed without tacking or supported with a binding wires.
Immediate action taken
The concern supervisor and the safety department was inform and he was taken to OCFZE refinery clinic for proper medical attention.
