Much like the US' VAERS reporting system, the EU has a EudraVigilance reporting system fro adverse events from vaccination. There are many doctors and hospitals have no clue about these reporting systems, let alone the need to use them. But cases do get reported.
In the US, a report was done a few years ago which shows that only 1% of adverse events actually get reported to VAERS. It may still be at that low number, although I think it's higher now, I still think they are heavily underreported.
As such, the number we get of vaccine injuries and deaths are going to be a lot higher than what we see. Nonetheless, we get a peak at how serious the issue is.

The EudraVigilance database reports that through June 19, 2021 there are 15,472 deaths and 1,509,266 injuries reported following injections of four experimental COVID-19 shots:
- COVID-19 MRNA VACCINE MODERNA (CX-024414)
- COVID-19 MRNA VACCINE PFIZER-BIONTECH
- COVID-19 VACCINE ASTRAZENECA (CHADOX1 NCOV-19)
- COVID-19 VACCINE JANSSEN (AD26.COV2.S)
Pfizer and Moderna have the highest deaths. AstraZeneca has the highest adverse events, about the total of Pfizer and Moderna combined, but less deaths than Moderna alone.

These are reported records, which are underreported, but still show that while AstraZeneca has received a lot of the spotlight in Europe, the new gene therapy injections carry a higher death rate it seems. Maybe that's because they are the most administered.
