En las remotas montañas de Colorado, el horror acecha dentro del manicomio de Mount Massive. Un hogar abandonado para enfermos mentales, que ha sido recientemente abierto para usos de “investigación y caridad” a manos de la transnacional corporación Murkoff, que ha estado operando en estricto secreto... hasta ahora. Siguiendo una pista de una fuente interna, el periodista independiente Miles Upshur, se adentra en las instalaciones y descubre una terrorífica línea entre la ciencia y la religión. Una vez adentro, él sabe que ya no podrá escapar, obligándolo así, a saber la verdad de lo que esta pasando en el corazón del manicomio de Mount Massive
Miles Upshur, un periodista independiente de un periódico de Denver, recibe un correo electrónico anónimo que le asegura que, en un Hospital Psiquiátrico llamado Mount Massive, están sucediendo sucesos extraños, desde que la Corporación Murkoff la compró hace un año.
Cuando llega a la entrada, se encuentra con numerosos camiones del ejército aparcados en la entrada. Al estar cerrada, Miles se ve obligado a retroceder y a ascender por varios andamios localizados a un lateral del edificio para poder entrar en el asilo
Una vez que Upshur logra acceder al interior del complejo, descubre a todos los guardias y agentes de la División Táctica de Murkoff masacrados en el interior de la biblioteca. El único superviviente, que agonizaba luchando entre la vida y la muerte, le ordena que salga del edificio, que si seguía allí, moriría. Miles hizo caso omiso de él y continuó su trayecto.
Stephenson, el último superviviente de la División Táctica de Murkoff, ordenándole salir cuánto antes del lugar.
Al salir de la biblioteca donde se encontraba el guardia, Upshur es atacado por un poderoso recluso llamado Chris Walker, quien lo arroja por una ventana, cayendo al vestíbulo del asilo Allí, se le aparece el Padre Martin Archimbaud, un sacerdote preso en el manicomio
Miles decide que es hora de salir del asilo, pero el Padre Martin le dice que no, que debe quedarse en él, ya que Dios se lo ha ordenado. Miles se recompone al cabo de varios minutos y se dirige a la sala de vigilancia, en donde comienza a recopilar más información
No obstante, el Padre Martin corta la electricidad, obligando a Miles a descender al sótano para restablecer la luz. Tras sortear a Chris Walker y a otro variante que se encontraba resguardando el piso inferior, Miles consigue volver a encender la electricidad, y regresa a la sala de ordenadores, para terminar de traspasar todos los documentos importantes sobre la Corporación Murkoff. Justo cuando está a punto de lograr descargar todos los archivos, es sedado por el Padre Martin, el cual no está dispuesto a que se vaya el periodista no sin antes mostrarle todas las atrozidades llevadas a cabo por Murkoff.
Miles despierta en una de las celdas de la prisión. Tras salir de la habitación, se encuentra al otro lado de unos barrotes a los gemelos. Éstos están dispuestos a devorar a Upshur, pero uno de ellos argumenta que no pueden todavía, pues está protegido por el sacerdote.
Upshur se abre paso a través del bloque de celdas, eludiendo a todos los antagonistas que se cruzan en su camino. Miles termina en las alcantarillas, donde vuelve a ser atacado por Chris Walker consigue escapar del variante y llega a la sala de lavandería accediendo al Pabellón Masculino
Allí es emboscado por el Doctor Richard Trager, un antiguo empleado de Murkoff que fue sometido a la terapia del Motor Morfogenético. Desde entonces, Trager recoge las partes del cuerpo de los internos e experimenta el dolor con ellos.

Trager encarcela a Upshur, lo ata a una silla de ruedas y corta dos de sus dedos con unas enormes tenazas. Upshur logra escapar y se produce una persecución, acabando con la muerte de Trager, aplastado por las puertas del ascensor
El Padre Martin se le volverá a aparecer, pidiéndole que se reúna con él en el exterior. Miles se abre paso a través de la cafetería, incendiada por Piro, y enciende los aspersores para apagar el fuego.
Ya en el exterior, Upshur sale a los terrenos de asilo, pero le persigue Walrider
Se ve obligado a dar la vuelta por la escalera en ruinas para acceder a las diferentes plantas del edificio, siguiendo un rastro de sangre que Martin dejó para que lo siguiera. Miles recorre todo el Pabellón Femenino, el cual se encontraba completamente vacío. Justo cuando está a punto de acceder a la entrada, el suelo cede y la videocámara cae por un agujero, obligando a Miles a descender para buscarla. Es atacado por tres variantes y los gemelos, antes de regresar al bloque de administración.
Upshur finalmente localiza a Martin en la capilla del asilo justo antes de presenciar la muerte de este, el cual se encontraba amarrado a una cruz. El Padre Martin le asegura que ahora que ya tiene pruebas suficientes para detener a Murkoff, Miles puede ser libre y él ya puede morir en paz, pues su misión ha concluido. Acto seguido, uno de los internos toma una antorcha y prende fuego a la cruz, acabando con la vida del sacerdote.
Upshur toma las llaves del ascensor para salir del asilo pero es engañado por Martin, puesto que en vez de llevarlo a la salida, el ascensor lo lleva a una instalación subterránea debajo del asilo. En la búsqueda de una salida
Upshur es atacado e incapacitado por Walker. Sin embargo, antes de que Walker pueda matarle, Walrider aparece y le asesinato brutalmente al recluso, incrustándolo contra una rejilla de ventilación
Upshur procede a investigar la instalación y se encuentra con el Dr. Wernicke (que originalmente se creía que estaba muerto), el científico responsable de todo el experimento conocido como "Proyecto Walrider"

Wernicke explica que el Walrider es resultado de los experimentos llevados a cabo con nanotecnología en un preso llamado Billy Hope, por lo que el Walrider es controlado por él
Ordena a Upshur encontrar a Billy en el Motor Morfogenético del laboratorio y matarlo apagando su sistema de soporte vital, para de esa forma también acabar con el Walrider. Después de realizar esta tarea, Upshur es capturado por el Walrider y es brutalmente golpeado, pero sobrevive mientras es poseído por éste. El Walrider desaparece y deja caer a Miles, el cual cae brutalmente contra el suelo, dejándolo cojo. Tambaleándose hacia la salida
se encuentra a Wernicke, acompañado por varios soldados de Murkoff armados que disparan repetidamente Upshur, cayendo al suelo. Mientras la pantalla se funde en negro, se escucha el sonido del Walrider atacando a los soldados, seguido de varios disparos por parte de los guardias.

In the remote mountains of Colorado, horror lurks inside the Mount Massive Asylum. An abandoned home for the mentally ill, it has recently been opened for "research and charity" uses by the transnational Murkoff Corporation, which has been operating in strict secrecy... until now. Following a lead from an inside source, freelance journalist Miles Upshur enters the facility and discovers a terrifying line between science and religion. Once inside, he knows he can no longer escape, thus forcing him to know the truth of what is happening in the heart of Mount Massive Asylum.
Miles Upshur, a freelance journalist for a Denver newspaper, receives an anonymous email assuring him that strange events are happening at a Psychiatric Hospital called Mount Massive since the Murkoff Corporation bought it a year ago.
When you arrive at the entrance, you find numerous army trucks parked at the entrance. When it is closed, Miles is forced to back up and climb up several scaffolds located at the side of the building to enter the asylum
Once Upshur gains access to the interior of the complex, he discovers all the guards and agents of Murkoff's Tactical Division massacred inside the library. The only survivor, who was dying fighting between life and death, orders him to leave the building, that if he was still there, he would die. Miles ignored him and continued his journey.
Stephenson, the last survivor of Murkoff's Tactical Division, ordering him to leave the site as soon as possible.
As he leaves the library where the guard was, Upshur is attacked by a powerful inmate named Chris Walker, who throws him out of a window, falling into the lobby of the asylum. There, Father Martin Archimbaud, a priest imprisoned in the asylum, appears to him
Miles decides it's time to leave the asylum, but Father Martin tells him no, that he must stay there since God has ordered him to. Miles composes himself after several minutes and goes to the surveillance room, where he begins to gather more information
However, Father Martin cuts the electricity, forcing Miles to go down into the basement to restore the light. After bypassing Chris Walker and another variant who was guarding the ground floor, Miles manages to turn the power back on, and returns to the computer room, to finish handing over all the important documents about the Murkoff Corporation. Just as he is about to download all the files, he is sedated by Father Martin, who is not about to let the journalist leave, but not before showing him all the atrocities carried out by Murkoff.
Miles wakes up in one of the prison cells. After leaving the room, he finds himself on the other side of some bars from the twins. The twins are ready to devour Upshur, but one of them argues that they cannot yet, as he is protected by the priest.
Upshur makes its way through the cell block, eluding all antagonists who cross its path. Miles ends up in the sewers, where he is attacked again by Chris Walker. He manages to escape from the bypass and reaches the laundry room by entering the Men's Pavilion
There, he is ambushed by Dr. Richard Trager, a former Murkoff employee who underwent Morphogenetic Motor therapy. Since then, Trager collects the body parts of the inmates and experiences the pain with them.
Dr. Rick Trager, moments before cutting off Miles Upshur's fingers. trager imprisons Upshur, ties him to a wheelchair, and cuts off two of his fingers with huge pliers. Upshur manages to escape and a chase ensues, ending with Trager's death, crushed by the elevator doors
Father Martin will appear to him again, asking him to meet him outside. Miles makes his way through the cafeteria, set on fire by Piro, and turns on the sprinklers to put out the fire.
Outside, Upshur goes out to the asylum grounds, but is chased by Walrider
He is forced to walk around the ruined staircase to access the different floors of the building, following a trail of blood that Martin left for him to follow. Thousands walk through the entire Women's Pavilion, which was empty. Just as she is about to enter the entrance, the floor gives way and the video camera falls through a hole, forcing Miles to descend to look for it. He is attacked by three variants and the twins, before returning to the administration block.
Upshur finally located Martin in the chapel of the asylum just before witnessing his death, which was tied to a cross. Father Martin assures him that now that he has enough evidence to arrest Murkoff, Miles can be free and he can die in peace, for his mission is over. Then, one of the inmates takes a torch and sets fire to the cross, ending the priest's life.
Upshur takes the elevator keys to leave the asylum but is tricked by Martin since instead of taking him to the exit, the elevator takes him to a subway facility underneath the asylum. In search of an exit
Upshur is attacked and disabled by Walker. However, before Walker can kill him, Walrider shows up and brutally murders the inmate, ramming him into an air vent
Upshur proceeds to investigate the facility and meets with Dr. Wernicke (originally believed to be dead), the scientist responsible for the entire experiment known as "Project Walrider"
Dr. Rudolf Wernicke and the guards before shooting Miles, wernicke explains that the Walrider is the result of experiments carried out with nanotechnology on a prisoner named Billy Hope, so the Walrider is controlled by him
Order Upshur to find Billy in the lab's Morphogenetic Engine and kill him by shutting down his life support system, thereby also killing the Walrider. After performing this task, Upshur is captured by the Walrider and is brutally beaten, but survives while being possessed by it. The Walrider disappears and drops Miles, who falls brutally to the ground, leaving him lame. Stumbling towards the exit
Wernicke is found, accompanied by several armed Murkoff soldiers who repeatedly shoot Upshur, falling to the ground. As the screen fades to black, the sound of the Walrider attacking the soldiers is heard, followed by several shots from the guards.
