Hola a toda la comunidad, hoy recibí una llamada de un paciente que manifiesta dolor en la zona del 3er molar.

Hello to the whole community, today I received a call from a patient who manifests pain in the 3rd molar area.

Al examen clínico se observa el capuchón eruptivo del 3er molar o cordal inflamado es lo que llamanos una pericoronitis, que es la inflamación del tejido periodontal que recubre la corona del diente, lo describe como dolor al masticar, a la apertura, al cepillado, pero en ésta ocasión era espontáneo e insoportable.

On clinical examination the eruptive cap of the 3rd molar or wisdom tooth is observed to be inflamed. This is what we call pericoronitis, which is the inflammation of the periodontal tissue that covers the crown of the tooth, and is described as pain when chewing, opening and brushing, but on this occasion it was spontaneous and unbearable.

Esto se puede deber a que el tercer molar intenta salir y el tejido que lo rodea es más fibroso que lo habitual, ésto tambien es muy común en lo dientes anteriores el paciente no permitió fotos pero les traigo unas de referencias.

This may be due to the third molar trying to come out and the tissue around it is more fibrous than usual, this is also very common in the anterior teeth the patient did not allow photos but I bring you some references.

El procedimiento se denomina Operculectomía que no es más que la eliminación de dicho tejido pericoronal con un bisturí, previa colocación de anestesia, en algunos casos el tejido esta tan inflamado e infectado por restos alimenticios que la anestesia hace poco efecto.

The procedure is called Operculectomy that noble more than the removal of such pericoronal tissue with a scalpel after placement of anesthesia, in some cases the tissue is so inflamed and infected by food debris that anesthesia makes little effect.

Luego de eso se realiza un legrado o limpieza de toda la zona le irriga con solución fisiológica y se deja abierta para que el diente erupcione de forma natural.

After that, a curettage or cleaning of the entire area is performed, irrigated with physiological solution and left open so that the tooth erupts naturally.

Debe tener los cuidados post operatorios de una cirugía menor, se le indica

• Tratamiento antibíotico y analgésico,
• Dieta líquida por 24 hora
• Dieta blanda por las siguientes 36 horas
• Tomar alimentos y bebidas frías
• No escupir ni hacer buches
• No molestarse la zona con la lengua
• No tomar bebidas con pitillo
• No tomar lacteos.

You must have the post-operative care of a minor surgery, you are instructed

• Anti-biotic and analgesic treatment
• Liquid diet for 24 hours
• Soft diet for the next 36 hours
• Eating and drinking cold foods and beverages
• No spitting or swallowing
• Do not bother the area with your tongue
• Do not drink drinks with a cigarette
• No dairy.

La erupción comenzará inmediatamente hasta lograr la salida de toda la corona dental. Es un procedimiento sencillo, de una duración aproximada de 30 min, de un post operatorio y un pronóstico muy bueno.

The rash will begin immediately until the entire crown comes out. It is a simple procedure, of an approximate duration of 30 minutes, of a post-operative and a very good prognosis.
