Los traumatismos dentales, son uno de los principales motivos de consulta de odontología, siendo los niños los más afectados por estos traumas existe una clasificación para poder abordar esta traumatismo, los diferentes tipos en los que se dividen generalmente los dientes que suelen ser más afectados por los traumatismos son los dientes anteriores superiores, estos suelen recibir el mayor daño, los traumatismos suelen ser más comunes en los niños pequeños de 2 a 3 años ya que debido a que son de comportamiento muy hiperactivos son más propensos a sufrirlos, en esta oportunidad le quiero mostrar una niña de 4 años la cual tuvo recientemente un traumatismo dental lo que la llevó a la consulta odontológa.
Dental trauma is one of the main reasons for dental consultations, with children being the most affected by these traumas there is a classification to be able to address this trauma, the different types into which are generally divided the teeth that are usually most affected by trauma are the upper anterior teeth, These usually receive the most damage. Traumatisms tend to be more common in small children between 2 and 3 years of age since they have a very hyperactive behavior and are more prone to suffer them, in this opportunity I want to show you a 4 year old girl who recently had a dental traumatism which took her to the dentist's office.
Al observar este traumatismo dental pudimos percatarnos que este era una intrusión dental, que a diferencia de algunos casos el diente sale por completo, pero esta vez el diente no salió no que se metió mucho más, existen dos posibilidades que el diente salga o esta segunda opción, debemos evaluar primeramente la severidad de la lesión para poder determinar en cual nos encontramos, conociendo su clasificación, como lo es la intrusión pacial I, esta se observa mas del 50% de la corona clínica, grado II intrusión parcial moderada, donde se observa menos del 50% de la corona, por último intrusión grado III intrusión severa, en esta no podremos observar ninguna parte de la corona, es decir, que el diente estará totalmente dentro del alvéolo dental.
On observing this dental trauma we could see that this was a dental intrusion, which unlike in some cases the tooth comes out completely, but this time the tooth did not come out but went in much deeper, there are two possibilities that the tooth comes out or this second option, we should first evaluate the severity of the lesion to be able to determine which one we are in, Knowing its classification, such as partial intrusion I, where more than 50% of the clinical crown is observed, grade II moderate partial intrusion, where less than 50% of the crown is observed, and finally grade III severe intrusion, where no part of the crown can be observed, that is, the tooth will be completely inside the dental alveolus.
Inicialmente la paciente llegó a consulta y tenía una semana posterior al traumatismo, la misma llegó con una radiografía panorámica y nos no se apreciaba bien el traumatismo por lo que se decidió hacer una radiografía coronal, en ella se permite determinar el tipo de traumatismo que tenía ya que nos permite ver de manera más detallada, la paciente era una niña de 4 años sumamente colaboradora, clínicamente no se evidenciaba ningún tipo de lesión hematoma cambio de coloración del diente, absceso o drenaje por lo que esto es una buena señal, se le mandó hacer esta radiografía y la semana entrante llegó para poderla evaluar efectivamente la niña se había dejado de tomar su radiografía coronal, en la misma se podía evidenciar que el traumatismo había tocado completamente el germen del diente permanente.
Initially the patient arrived for consultation one week after the traumatism, she arrived with a panoramic x-ray and the traumatism was not well appreciated so it was decided to do a coronal x-ray, it allows us to determine the type of traumatism she had since it allows us to see in a more detailed way, the patient was a 4 year old girl who was very cooperative, Clinically there was no evidence of any type of lesion, hematoma, change of color of the tooth, abscess or drainage, so this is a good sign, she was sent to have this x-ray done and the following week she arrived to be able to evaluate her, in fact the girl had not taken her coronal x-ray, in the same one it could be seen that the trauma had completely touched the germ of the permanent tooth.
Ya una vez cuando se conoce el tipo de traumatismo dental, que en este caso fue una intrusión debe planificar el plan de tratamiento más idóneo, para nuestra paciente en este caso la odontopediatra llegó a la conclusión de extraer el diente 51, debido a que este se encuentra afectando el germen dental del diente permanente que en ese caso sería el 21, pero constantemente se debe realizar un registro radiográfico en el paciente cada 6 meses, para poder observar cada una de las estructuras del diente y si esta tienen alguna repercusión en caso de no extraer el diente, se debe esperar que el paciente vaya a tener su proceso de rizalisis, esperar que el tiempo vaya hacer su recambio y que el diente vaya bajando se debe realizar cada 6 meses, esta radiografía para evitar que el diente permanente se siga comprometiendo la odontopediatra decidió realizar la exodoncia de la unidad dentaria 51, no obstante luego de realizar esta extracción se debe colocar un mantenedor de espacio, para guardar el espacio del futuro diente permanente.
Once the type of dental trauma is known, which in this case was an intrusion, the most suitable treatment plan should be planned, for our patient in this case the pediatric dentist came to the conclusion to extract tooth 51, due to the fact that it is affecting the dental germ of the permanent tooth, which in this case would be tooth 21, but a radiographic record should constantly be taken of the patient every 6 months, to be able to observe each of the structures of the tooth and if this has any repercussions in the case of not extracting the tooth, It is necessary to wait for the patient to go through the process of rhizalysis, wait for the time to make its replacement and for the tooth to go down, it should be done every 6 months, this x-ray to avoid the permanent tooth to continue compromising the pediatric dentist decided to perform the exodontia of the dental unit 51, however after performing this extraction a space maintainer should be placed, to save the space of the future permanent tooth.
Por último el tiempo que se transcurre después del traumatismo es vital, ya que de él va a depender el pronóstico del tratamiento, para el traumatismo dental muchas veces algunas personas dejan esperar tanto tiempo, esto compromete los dientes es por eso que inmediatamente, al momento de ocurrir un traumatismo dental debes llevar a tu niño o al odontólogo para que éste tenga la conducta adecuada a seguir, así poder tener el mejor tratamiento para el paciente conservando el diente en boca, si es necesario y abordar al paciente cabe destacar que cuando existe compromiso pulpar se debe abordar de manera inmediata ya que en este caso el pronóstico del diente en boca va ser reservado.
Finally, the time that elapses after the trauma is vital, as the prognosis of the treatment will depend on it. In the case of dental trauma, some people often wait too long, which compromises the teeth. That is why, immediately after dental trauma occurs, you should take your child to the dentist so that they can take the appropriate action This will ensure the best treatment for the patient, preserving the tooth in the mouth if necessary. It should be noted that when there is pulp involvement, it must be addressed immediately, as in this case the prognosis for the tooth in the mouth will be uncertain.
