Paciente de femenina de 12 años de edad asiste a consulta odontologica, con gran preocupación ya que solamente le han erupcionado los dientes permanente inferiores y el incisivo central superior derecho que corresponde a la unidad dentaria 11, por lo que no ha podido hacer su recambio dentario de los dientes temporales, mediante la radiografías panorámica pudimos evidenciar que no posee exfoliación de las raíces de los dientes temporales, así como también no posee suficiente espacio para la correcta erupción de los dientes permanentes, es un caso bastante complejo por lo que se manejo de manera multidisciplinaria con las especialidades de odontopediatria y ortopedia funcional de los maxilares.
A 12 year old female patient comes to the dentist's office with great concern because only the lower permanent teeth and the upper right central incisor that corresponds to dental unit 11 have erupted, so she has not been able to replace her primary teeth, By means of the panoramic radiographs we were able to show that she does not have exfoliation of the roots of the primary teeth, as well as not having enough space for the correct eruption of the permanent teeth, it is a very complex case so it was handled in a multidisciplinary manner with the specialties of pediatric dentistry and functional orthopedics of the jaws.
Cuando hablamos de retención prolongada de dientes deciduos, diente retenidos se refiere aquel diente que se queda justamente en los maxilares y estos no logran erupcionar como es debido se quedan en el hueso completamente, en este caso particularmente se debe a multiples causas la paciente posee antecedentes de enfermedades renales, por lo que los dientes retenidos se asocian a multiples enfermedades renales, la paciente solo presentaba pequeños calculos renales pero esto también puede tener estrecha relación con la retención de dientes deciduos.
When we speak of prolonged retention of deciduous teeth, retained tooth refers to the tooth that remains precisely in the jaws and these are unable to erupt as they should, they remain in the bone completely, in this case particularly due to multiple causes, the patient has a history of kidney disease, so the retained teeth are associated with multiple kidney diseases, the patient only had small kidney stones but this may also be closely related to the retention of deciduous teeth.
En esta paciente los dientes retenidos tienen multiples causas, como se puede evidenciar mediante el estudio radiografico la paciente no posee suficiente espacio para la erupción adecuada de los dientes permanentes, es decir los caninos y premolares permanentes no presentan suficientes espacios para erupcionar, así como también el incisivo central superior izquierdo que corresponde a la unidad dentaria 21, presenta la raíz curva y también esta corta, así como también estos dientes algunos se encuentrar giroversionados es decir que vienen en una posición que no es la pertinente ya que eso afectara la oclusión dentaria en la paciente por completo.
In this patient the retained teeth have multiple causes, as can be seen in the radiographic study the patient does not have enough space for the adequate eruption of the permanent teeth, that is to say the permanent canines and premolars do not have enough space to erupt, as well as the upper left central incisor that corresponds to the dental unit 21, has a curved root and is also short, as well as some of these teeth are giroversionados that is to say that they come in a position that is not relevant as this will affect the dental occlusion in the patient completely.
Cabe destacar que la paciente después de sus cuatros años de vida fue que inicio una alimentación fibrosa es decir, no ingeria carne, pollo, nada solido, por lo que esto también pudo ser la causa de que las raíces de los dientes deciduos no se encuentren exfoliados ya que esto genera como consecuencias, que los dientes primario sigan en boca, ya que los dientes permanentes debido a esto no erucpcionan, es un caso realmente muy interesante, resulta necesario tener una alimentación fibrosa ya que esto contribuye al desarrollo transversal de la mandíbula.

Por último para finalizar cabe destacar que ya a los 12 años no deberían estar los dientes temporales en boca, si esto ocurre debemos asistir cuanto antes al odontologo para conocer la causa de esto verdaderamente, es primordial la radiografía panorámica para un excelente diagnóstico así como el manejo de las distintas especialidades de la odontología, importante conocer que para realizar las exodoncias seriadas de los dientes temporales se debe planificar de manera muy cuidadosa el caso ya que si no lo realiza un especialista altamente capacitado podemos originar como consecuencia que los dientes permanentes se impacten.
