The birth of a child is a great event, everyone is happy, in general that is what happens. The couple hopes that everything will go well, that it will evolve satisfactorily during the 9 months until the moment of seeing it in person arrives. No matter how much 3D ultrasound is used nowadays, it never comes close to that magical moment of birth.
However, not everything always turns out well, unfortunately, and losses often occur, which is the counterpart, it is a totally traumatic event for most families. But an abortion is something that can eventually occur even without apparent cause, and it is something that we as physicians must learn to deal with, which I must say is not easy at all.
I would like to talk about abortion, not the one that is provoked, but the one that happens for no reason at all. It is a topic that is not often talked about, but it is a big problem, because it additionally compromises the mother's life, which makes the situation even more complicated.
As is well known, a pregnancy tends to last an average of 38 weeks. A miscarriage occurs when there is a loss before the 20th week. It often happens that the woman does not even know she is pregnant, although this seems hard to believe.
It could be intuited that some activities that the woman performs while pregnant without knowing it could produce a miscarriage, but it usually happens that 3 out of 10 women can have a miscarriage, even knowing it and taking care of themselves in the process, this leaves room for an endless list of possibilities as causes of this outcome.
These women usually have bleeding that is greater than usual in terms of quantity, but they assume it is part of the menstrual process, some uncontrolled or other reason, and as with the passage of days disappears most of the time, they simply do not realize that they were pregnant.
Generally, it is said that the main cause of a miscarriage is that the embryo did not develop properly, and has some abnormalities due to genetic problems, which causes the body to expel it because it considers that the chances of living are limited.
Although cases can be seen in which in spite of having genetic abnormalities, they are born and live even for years. ***Everything is relative in this aspect *** As I always say, in medicine 2 plus 2 is not four.
There are also explanations that are related to uterine problems, there are women who are called "habitual miscarriers ", which are those women who for unspecified reasons have 3 or more miscarriages constantly.
There are many factors, as I have already mentioned, even a hormonal disorder can provoke it. Keeping in mind that embryonic development requires a perfect balance in the concentration of hormones in the woman.
This subject has always seemed to me a bit complex to deal with, because the emotional aspect is always very deeply involved, especially when it is a child that has been conceived consciously, that is desired, and it is here that the psychological aspect comes into play.
In addition, there is the aftercare, because unfortunately there are many problems, especially infections. Even more so when they are induced abortions, some of them using substances that do not guarantee the slightest safety for the woman. I have particularly seen women die from infections that ended up being systemic, that is to say, that ended in sepsis, and the problem is that out of fear they opt for unconventional measures to terminate a pregnancy.
This is a legal aspect, of course, but the medical act should never be separated from the legal, because all medical action must be within a medical legal framework, as well as an ethical and penal framework, which varies greatly from one country to another.
But, returning to the subject, the risks that a woman runs in an abortion are many, but the main ones I could say are the following:
Hemorrhage: this to a great extent is going to depend on the stage of embryonic development in which it occurs and also on the cause of the same. Keep in mind that the uterus is a highly vascularized organ, the uterine artery is a direct branch of the abdominal aorta, so any injury that occurs in this organ hemorrhage is very difficult to stop.
Infections: if the abortion is incomplete, and there are embryonic remains at the uterine level, or if the abortion was not spontaneous but was done by non-medical means, it can also trigger an infection with an internal starting point that is very difficult to treat.
I could say that physically these are the main physical consequences, but beyond this there are also very severe psychological effects on the woman who goes through such a situation, which is not much talked about, but it is something that is present.
Maternal health is something that should always be considered as something relevant to any country, unfortunately I have noticed that the negative figures of this problem are increasing, especially in developing countries. It is an issue with many edges, to tell the truth, but I think we can all do something to prevent complicated cases continue to occur, and with very basic education on this issue much can be done, I'm sure that if.
