We are open to suggestions regarding ways to improve the process.

We discussed asking for an email address, so that we can send a confirmation. However, we thought some folks might be hesitant to give us their email address. I guess we could make that optional, i.e. "If you want an email notification confirming your submission, please provide an email address."

Any thoughts about that?
