En esta ocasión les cuento las actividades especificas que realiza el personal de enfermeria en mi servicio de hospitalizacion de medicina interna desde el comienzo del turno hasta finalizar, en el turno por lo general suelen haber dos enfermeras en donde distribuimos funciones para cada una y facilitar el trabajo en equipo. Sin embargo en ocasiones pueden haber mas de dos enfermeras debido a que se puede contar con pasantes de enfermeria nuestro turno comienza desde las siete de la mañana hasta la una de la tarde.
Al comenzar el turno realizo un recorrido por todos los aislamientos del servicio principalmente para observar el estado actual del paciente con un pequeño examen fisico, de igual forma es importante el interrogatorio entre el paciente y familiar asi podre recolectar informacion importante sobre su estado de salud, luego de esto realizamos el pedido de medicamentos para 24 horas de cada paciente.
Luego acostumbro a realizar las extracciones de muestra de sangre por paciente esto deberia realizarse a primera hora de siete a ocho de la mañana, a esta misma hora se realiza el control de glucemia capilar la mayoria de los pacientes hospitalizados son diabeticos y de acuerdo el valor de glucemia se procede a cumplir el esquema de insulinas cristalina y NHP, si ocurre lo contrario y el paciente esta presentando hipoglucemia le notificamos al medico de guardia para comenzar el tratamiento correspondiente.
A las nueve de la mañana se comienza a administrar el tratamiento indicado a cada paciente, tambien podemos cateterizar las vias periféricas dañadas o que tengan un lapso de tres dias es fundamental para la prevencion de infecciones, posteriormente realizamos las curas de cateter venoso central un día si y otro no, la administracion de hemoderivados tambien las realizamos en las mañanas.
A la hora del medio día enfermeria debera realizar el control de glucemia capilar antes del almuerzo de igual forma dependiendo el valor arrojado cumplimos el esquema de insulina o no, al finalizar el turno si hay actualizaciones escribimos los pendientes como pruebas de diagnosticas, hemoderivados, entre otros procedimientos médicos, escribimos la evolucion de enfermeria por cada paciente. Otras de las actividades destacadas es el llenado del libro de enfermeria en donde escribimos el personal por turno, ingresos, egresos, pacientes de cuidado, fallecidos, traslados, camas y equipos con que cuenta el servicio.
Tambien es importante organizar el servicio como lo es el carro de tratamiento, contar el material y el equipo con que se dispone, dejar las historias acomodadas en un solo lugar, equipar el carrito de tratamiento con el material disponible, finalizamos con la entrega del turno al personal de guardia siguiente.
