I'm a mix of "over-prepared" and "on the fly". I tend to start to prepare early, with notes and maps and NPCs and other things - then suddenly it's the day before playing and I'm "Oh no, I have no idea what to do!"
Good that I have all the prepared stuff and a fast working mind ;)
