(This post is also written in English) Como viene siendo costumbre, rotación terminada, impresiones que dejamos por aquí.
En el artículo de hoy, te cuento, como me fue en mi rotación por la especialidad de Psiquiatría. Un resumen de lo que fueron mis cinco semanas en una rotación más.
Si eres nuevo en mi blog. Soy estudiante de medicina, en Cuba. Ser estudiante de medicina, no es fácil en estos momentos (nada, es fácil) El sector de la salud ha vivido una migración y éxodo, solo comparado con el que ha vivido el país a nivel general.
En este panorama, yo trato de terminar mi carrera, preparándome lo mejor que pueda.
La rotación de psiquiatría no era de las que más he esperado, ni la que más me emocionaba, solo una más
Aunque siempre queda la emoción de empezar algo nuevo.

As usual, rotation completed, impressions we leave behind.
In today's article, I'll tell you how my rotation through the Psychiatry specialty went. A summary of what my five weeks in another rotation were like.
If you're new to my blog, I'm a medical student in Cuba. Being a medical student is not easy at the moment (nothing is easy) The healthcare sector has experienced a migration and exodus, only comparable to what the country has experienced in general.
In this panorama, I try to finish my degree, preparing as best I can.
The psychiatry rotation was not one of the ones I was most looking forward to, nor the one that excited me the most, just another one.
Although the excitement of starting something new always remains.
Primera Semana
El ala de psiquiatría es una de las más apartadas de nuestro hospital (por razones obvias)
En mis guardias en el servicio de urgencia, nunca he tenido la oportunidad de entrar a esa ala y durante la carrera, mucho menos. La curiosidad siempre me la he tenido.
La primera semana, era una breve toma de contacto con la rotación.
Se presentaron los profesores principales (más nunca los vi) y los residentes, que se encargarían de los estudiantes durante la estancia en la sala.
50 alumnos para 10 camas (matemáticas simples: 5 alumnos por cama, no salíamos ni a un miembro por estudiante)
Los pases de visita, se hacían solo frente a tu paciente y las discusiones diagnósticas sin conocer al resto de pacientes (había que imaginárselos)
¡Desilusión total!
De esta semana solo salvo, la actitud de los residentes. Siempre estuvieron en disposición de enseñarnos y de verdad, nos dieron unas clases que no tuve que estudiar esos temas más nunca.

First Week
The psychiatry ward is one of the most remote in our hospital (for obvious reasons)
In my shifts in the emergency department, I've never had the opportunity to enter that ward and much less during my degree. I've always had the curiosity.
So the first week was a brief contact with the rotation.
The main professors (whom I never saw again) and the residents who would be in charge of the students during the stay in the ward were introduced.
50 students for 10 beds (simple math: 5 students per bed, we didn't even go out to one member per student)
The ward rounds were done only in front of your patient and the diagnostic discussions without knowing the other patients (you had to imagine them)
Total disappointment.
From this week, I only saved the attitude of the residents. They were always willing to teach us and truly gave us classes that I never had to study those topics again.
Segunda Semana
Seguíamos en la Sala, así que más de lo mismo.
Nos sucedió algo que no olvidaríamos ...
Llegó un señor, con sus años vividos y una bata más blanca que la luna llena. Asumimos que era profesor principal y él dijo que empezaría el pase. Todo muy normal.
El recién llegado profesor no hacía muchas preguntas, así que nada de quejarnos. Sería profesor si hubiese estudiado alguna vez en la carrera.
Resulta que era un paciente más de la sala y nos habían hecho una broma, entre pacientes y residentes -Nadie se dio cuenta.
En cierto modo, esta broma, nos enseñó hasta qué punto ciertos pacientes pueden llevar sus patologías y simular ciertas situaciones en su vida.
Las dos semanas se fueron volando. También, solo íbamos por las mañanas y la tarde era para nosotros.
Esta es una de las mejores cosas de la rotación. Ocupar la tarde de los estudiantes con asignaturas complementarias, que no aportan nada, es lo peor que hace la universidad.

Second Week
We continued in the Ward, so more of the same.
Except that something happened to us that we would never forget...
A gentleman, with his years lived and a gown whiter than the full moon, arrived. We assumed he was the main professor and he said he would start the rounds. Very normal.
The newly arrived professor did not ask many questions, so no complaining. He would be a professor if he had ever studied in the degree.
It turns out that he was another patient in the ward and they had played a trick on us, between patients and residents. No one noticed.
In a way, this prank taught us the extent to which certain patients can carry their pathologies and simulate certain situations in their lives.
The two weeks flew by. Also, we only went in the mornings and the afternoon was for us.
This is one of the best things about the rotation. Occupying the students' afternoon with complementary subjects that contribute nothing is the worst thing the university does.
Tercera Semana
Nos tocaba ir hacia psiquiatría pediátrica.
El centro no tenía las condiciones para recibirnos, éramos más alumnos que sillas. Solo había un profesor para atendernos.
Solución: Le damos las clases en un día y se las evaluamos al final de la semana
Seguían las desilusiones. Cuando cogí mi carrera de medicina, esperaba estar más tiempo en el hospital que en mi casa.
Ahora resulta que me paso más tiempo en mi casa, que en las rotaciones que me tocan.
No me quejo mucho, porque me permite tener un horario más flexible y hacer otras cosas que igual me gustan, pero ¿qué médico puede formarse estando más tiempo en su casa que viendo pacientes? ¡Cosas de Cuba!

Third Week
We had to go to child psychiatry.
The center did not have the conditions to receive us, there were more students than chairs. There was only one professor to attend to us.
Solution: We give the classes in one day and evaluate them at the end of the week.
There were more disappointments. When I took up my medical career, I expected to be in the hospital more than at home.
Now it turns out that I spend more time at home than in the rotations assigned to me.
I don't complain because it allows me to have a more flexible schedule and do other things that I also like, but what doctor can be trained spending more time at home than seeing patients? Things of Cuba.
Cuarta y Quinta Semana
Volvíamos a cambiar de escenario docente.
Esta vez, nos asignaban al Centro de Salud Mental de nuestro municipio: Plaza de la Revolución
De nuevo ... no había condiciones para nosotros.
Solución: Nos asignaron un paciente, que debíamos visitar, entrevistarlo y hacer un informe.
Mi experiencia:
La dirección que me dieron estaba incompleta. (El centro de salud no tiene control sobre los pacientes)
Encontramos el consultorio del médico de la familia (sí, nadie nos orientó, tuvimos que pedirle ayuda a los vecinos) El médico de la familia, ni sabía de quién hablábamos.
Al fin llegamos a la casa y la familia del paciente, no sabía nada de nuestra tarea. (Horas antes, nos habían dicho que ellos habían informado de nuestra visita. )
No solo nos mintieron, sino que demuestran la mala comunicación entre el centro y sus pacientes. Un desastre, nada que nos asombre ya.
Hicimos nuestro trabajo y la entrevista lo más profesional posible y hasta café nos invitaron, pero mucho que desear del centro de salud.
Para culminar la rotación, nos hicieron una prueba. La misma prueba que le hacen a todas las rotaciones. Variaron algunas preguntas, pero el resto no.

Fourth and Fifth Week
We were changing educational setting again. This time, they assigned us to the Mental Health Center of our municipality: Plaza de la Revolución.
Again, there were no conditions for us.
Solution: They assigned us a patient, whom we had to visit, interview and write a report.
My experience:
The address they gave me was incomplete. It shows that the health center has no control over the patients.
We found the office of the family doctor -yes, no one guided us, we had to ask the neighbors for help- The family doctor didn't even know who we were talking about.
Finally we arrived at the house and the patient's family didn't know anything about our task. A few hours earlier, they had told us that they had informed them of our visit.
Not only did they lie to us, but they also demonstrate the poor communication between the center and its patients. A disaster.
We did our work and the interview as professionally as possible and they even invited us for coffee, but a lot to be desired from the health center.
To culminate the rotation, they gave us a test. The same test they give to all the rotations. They varied some questions, but the rest did not.
You can imagine the result of the test.
Cinco semanas, perdidas. Las enseñanzas que me llevo de esta rotación, son menos que nula.
Una pena, porque la Psiquiatría tiene mucho para enseñar y más en un país que se está viviendo con tanto stress y donde las drogas van tomando las calles. Es la realidad que vivimos.
A lo largo de mi carrera he pasado por rotaciones, pero las de quinto año, se superan una tras otra.
Lo único que me llevo de esta rotación y más que una enseñanza es una máxima es... Cuida tu salud mental sobre todas las cosas.
Una vez que perdemos esta, la calidad de vida nuestra y de nuestros seres queridos se ve disminuida grandemente. Así que cuídate y cuida a tu familia.
Por suerte, esta rotación, ya está en el pasado.
El tema de la salud mental, es uno de los más abordados y a la vez evitados por las personas... Tú qué crees de ello, ¿Consideras que los sistemas educacionales y de salud deberían darle más prioridad?

Five wasted weeks. The lessons I take away from this rotation are less than null.
A pity, because Psychiatry has a lot to teach and more in a country that is living with so much stress and where drugs are taking over the streets. It's the reality we live in.
Throughout my career I have gone through rotations, but those of the fifth year surpass each other one after the other.
The only thing I take away from this rotation and more than a lesson is a maxim is... Take care of your mental health above all else.
Once we lose this, the quality of life of ourselves and our loved ones is greatly diminished. So take care of yourself and your family.
Fortunately, this rotation is already in the past.
The issue of mental health is one of the most addressed and at the same time avoided by people... What do you think about it, do you consider that the educational and health systems should give it more priority?
